Job Title: Clinical Pharmacist I
Location: Hawthorne, NY 10532
Schedule: Monday Friday, 9:00 AM 5:30 PM (occasional flexibility as needed)
Assignment Duration: 12 months (no extension or conversion)

Position Summary

Seeking experienced Clinical Pharmacists to support fulfillment operations and clinical services during the implementation of new pharmacy software (SM Cloud). This role combines order verification, patient counseling, and collaboration with the fulfillment team to ensure timely, accurate, and compliant medication dispensing.

Key Responsibilities

• Perform second verification (PV2) of prescription orders, ensuring accuracy in dosage, drug selection, interactions, route, and stability.
• Process an average of 18 PV2 orders per hour in a fulfillment environment.
• Conduct clinical patient counseling calls, addressing therapy, side effects, adherence, and safety concerns.
• Review patient charts for completeness, including allergies, medical history, lab results, and concurrent medications.
• Provide clinical interventions and assess patient responses to therapy.
• Collaborate with physicians, nurses, and patients to resolve clinical and technical issues.
• Supervise and guide Pharmacy Technicians, Interns, and other pharmacy staff.
• Maintain compliance with all regulatory standards, state/federal requirements, and company SOPs.
• Participate in performance improvement projects and in-service programs.
• Review relevant literature and remain current on drug therapy practices.
• Maintain accurate documentation of all clinical interactions.

Required Qualifications

• Education: Bachelor's degree in Pharmacy (required); Pharm.D. preferred.
• Licensure: Active New York Pharmacist license (required). Ability to obtain additional state licenses preferred.
• Experience: Minimum 5 years of pharmacy experience in mail order, retail, or hospital settings.
• Strong collaboration, multitasking, and quality-focused work ethic.
• Willingness to work in a team-based, fast-paced environment.

Preferred Qualifications

• Multi-state pharmacist licensure.
• Experience in specialty pharmacy or high-volume fulfillment settings.
